health organization,

we realize that tackling health disparities together is the only way to create a worry-free society where all individuals have access to equitable healthcare. Therefore, we are committed to helping bridge the health disparity gaps by offering skill-building opportunities to increase access to care and partnering with healthcare professionals. By leveraging their expertise, we can conduct research and ensure evidence-based patient-centered care, which will ultimately help optimize healthcare costs while elevating the quality of care overall. It’s an ambitious journey, but one in which we stand united and remain enthusiastic about embarking on!

Programs

To facilitate the national goals of achieving health equity and patient-centered outcomes based care, we developed programs that focus on health coverage and access, clinical care solutions for the underserved, promoting health literacy, and empowering patients and caregivers to provide feedback to the health care entities on what matters most to them.

This model points out that the contribution of clinical care is less than 20% on the health outcomes. However, per capita healthcare cost amounts to 14,000 USD per year which is equal to 100% of the Federal Poverty Level (FPL). This burden of healthcare costs ensures the unsustainability of today’s healthcare system. The framework also points out opportunities for the healthcare stakeholders to design solutions around the health factors that contribute 80% towards the health outcomes.Therefore, our programs are designed to consume healthcare resources proportionately as evident in the framework. Each of the four program designs is based on the logic model that is community-driven.
